NHS investigations not thorough

The health service ombudsman has said that many of the NHS investigations into allegations of avoidable harm or death are not sufficiently thorough.

Dame Julie Mellor said that she found that more than a third of 100 investigations were inadequate.

She told the BBC News Channel that “it is not acceptable” that allegations of avoidable harm are not properly investigated.

Since my motorbike incident strange things have been happening in my life and I have been talking to doctors and health professionals about my recollections of them in detail.

National guidelines state record keeping should be factual, accurate and consistent. In my opinion the record keeping is diabolical
